1. The email/paper application is attached. Read instructions carefully as the Request for DCAT Funds directions and these email directions have been changed as of 4-22-2015. If you do not return the documents to us in the way and the order specified with these updated changes (with all parts complete), they will not be considered for scoring.
2. Send your email application as a Word Document only to me as (1) one email attachment ([email protected]) by 430pm May 11th, 2015 and (2) mail seven stapled paper copies (flat) that must be in my mail flow by 430pm May 12th, 2015. Send paper copies to: Joe Burke, % DHS, Box 729 Indianola Iowa. 50125. You do not need to electronically sign the email attachment. You may drop off the paper copies at the DHS Office at: Warren County Department of Human Services 1005 South Jefferson Way Indianola, Iowa 50125
3. Do not separate out the Matrix as a separate document when you email the proposal to me.
4. Send only the pages required in the order required and not the entire application. Make sure you include a phone number on your application where I can reach you as quickly as possible (cell phone or person’s phone that knows how to get ahold of you) as our time frame is very tight and I may need information from you quickly in regards to your application.
Preparing Your Proposal
1. Print out this email for future reference
2. Open the Request for DCAT Funds Email Application and click “save as”
3. Save it to a word file
4. Once saved click “save as” again
5. This time save it with a new name: Name of your Project, the county(s) it will serve, “FY16” and your amount requested. Abbreviate as needed.
6. Now delete all the pages but 6,8-10 from this second document and hit “save”
7.You now have a document for reference, directions, etc. and a working document for your proposal in the proper order and format.
Scoring/Awards
1. This year the CPPC/DCAT Steering Committee Voting Members, CPPC Coordinator, Sarah Hohanshelt, as well as Joe Burke DCAT Coordinator, have asked an expert from the State of Iowa to assist us with the review and scoring of our Mini Grants. This will be done in a process called “Consensus”. The CPPC Steering Committee will review applications before the two consensus meetings in June. They will list what they consider “Strengths and Weaknesses” of your proposal and bring that to the “Consensus” meeting for review. Through the process of discussion, one score for your application will then be agreed upon by the Steering Committee and sent to the DCAT Governance Board for our counties. The five DCAT Goals on page two of the attached Request for DCAT Funds Application will be what your strengths and weaknesses (including your budget) will be measured against. Your one score will then be sent to the DCAT Governance Board for their consideration and action. All those who put in an application will be informed of the awards in mid to late June 2015. Madison County will be the Contract Holder and all grant award agencies will be sub- contractors, based upon their proposal, to the Contract.

f) June 13-15 Joe Burke develops Contracts with Madison County Board of Supervisors as the Contract Holder.
g) June 16-17 DCAT Board reviews Contracts.
h) June 18 DCAT Phone Board Meeting at 830am to approve Contracts. DCAT Board Chair signs Contracts.
i) June 19 Contracts sent before noon to Madison County Board of Supervisors for review.
j) June 23 Madison County approves contracts. Madison County Board of Supervisors Board Chair signs Contract.
k) June 23 Contract sent to Area 5 DHS Service Area Manager for signature and then on to the State.
l) July 1 Contracts begin.
2. There is no guarantee that you will be awarded money and all of the money available does not have to be allocated. The DCAT Board reserves the right to raise or lower any funding request. The projects can be for one of our counties or they can be for multiple counties in our Indianola DCAT Cluster. The projects/proposal can be for programs that already exist and we are supplementing a current budget or to expand it, increase its quality, etc. or it can be a new project.
FYI: State Secure Encryption Tumbleweed System
1. Throughout the year you may be receiving messages and attachments from workers on the State of Iowa system in addition to this one. If it comes to you on the secure encryption State Tumbleweed System (easily recognizable) it will only stay in your email system (including attachments) for about 30 days.
2. If you want to save the text of the email and/or attachments you must copy the email text and save it to a word doc in your own computer or click “save as” for attachments.
3. All attachments are usually at the very top or bottom of the page when you receive a secure encryption State Tumbleweed email.
